What the governance self-assessment measures

Review 15 practical questions about quality and risk, records and accountability, and feedback and improvement. For each question, note whether the arrangement is absent, partly in place or supported by current evidence. This can also be done as a written evidence-and-action list.

The tool gives equal weight to every answer: not in place scores 0, partly in place scores 1 and evidenced scores 2. The total is out of 30. There is no pass mark, and a high total does not show that a serious gap elsewhere is acceptable.

An evidenced answer records your own view that suitable evidence exists; an auditor has not checked it. Use the result to identify missing evidence, an accountable owner and the next review needed. The score is not a CQC rating or a finding of legal compliance.

Answers remain in the page while you use the tool. A summary is downloaded only at your request. Use references to records rather than patient names, care details or identifiable staff concerns.
